To help your first visit go smoothly, we ask that you complete our New Patient Form ahead of time and have your dental records ready. If you’ve had treatment elsewhere, you can either bring your records with you or request that your previous dentist send them to info@acfrd.com.

Completing the form in advance helps us get to know you and your dental history so we can focus on your care right away.
